LM-79 TEST REQUEST FORM

Please use this form for tests covered by LM-79 standard, including Goniophotometry and Integrating Sphere tests for LED luminaire.. This form should be used only for LED luminaires and lamps.  Please do not use this form for non-LED luminaire or bulbs.
